This is not a known problem.

Which version of Server ?  And if you've replaced the MySQL that
comes with it, which version of MySQL ?  And how are you adding users ?

We have mysql running on another XServe without Xsan and don't have
any problems there, so I think that either Xsan or mysql is
misconfigured for this particular environment.

Other possibilities: corrupted binaries; one or more of the files SQL maintains is protected against SQL; hard disk sector failure.
