Simplest way that I know of is to boot off the installation DVD and
use the reset password tool from the menu.
Yes. It's even the documented method for performing this task.

Then perhaps what you're seeing is that you're confusing domains in
OD or your OD is broken or misconfigured.
Is this a standalone domain?
