Are you authenticating for smtp ? (should be). As well, in OS X Server by default, spamassassin is called & controlled by amavisd, where
you can setup whitelisting as needed/desired.

For the sake of thoroughness, please share the un-obfuscated output of:

postconf -n

I do have everyone authenticate for smtp.  Is there then a way to not check for spam if mail has been authenticated?
