I did that exact same thing and there was no "None" button.
I got really frustrated and just put in a school card.

I just started through the process on a machine with iTunes and no account.  I went to the iTunes Store, then the App Store, and then selected a free app to download.  It gave me the option of signing in or creating an account.  I chose to create an account.  When I got to the screen about payment information, None was an option that I could select. So Apple does still appear to support that method for creating accounts without a credit card.

Creating an Apple ID on a computer

1.    Open iTunes. If you do not have iTunes, you can download iTunes here.
2.    From the Store menu, choose Create Account. Click Continue on the resulting screen.
3.    To proceed, you'll need to agree to the Terms and Conditions. Once you've read and agreed to the Terms and Conditions, click the checkbox next to "I have read and agree to these terms and conditions."
4.    Click Continue. 
5.    Complete the form to create your free Apple ID (which is what you will use to log in to the iTunes Store).
6.    Provide a valid payment method and billing address. You will not be charged until you make a purchase.
7.    Once you've provided all the required billing information, click Continue.

I think you need to try it the opposite way.  Without being logged in, go to the App Store and try to download a free app.  It should then ask you to log in or create an account.  When you create the account at this moment, None is available as a payment option.

I can't even log in. It just keeps saying "The Apple ID has not been used with iTunes store. Please review account information" then I have a cancle or review button. If I hit review I get into the credit card part

I have an apple id already for our school. I used it to order a server and a few ipads. It has never been set up in iTunes or The Mac Appstore. I now have a Lion server and want to use it with iPads. So I am trying to use that ID for that purpose.

Well, I go through the set up again, and I get to the billing section and there is no "none" option.
 
So how do I set up an account in iTunes (and then in the Mac App Store) without a credit card?
