Thanks! I now have custom paper sizes working. A few oddities to get my head around with the margins but the paper size is definitely correct. What are the odds of the printing architecture handling a 150 foot by 60 inch print job? ;-)

Do not use the custom paper size definitions in the currently shipping headers. They will not work. You must use the defines that I put in my Feb 21 email (see below). Like I said, a future release of the Developer Tools CD will contain the correct PMTicket.h and PMTemplate.h. Sorry for the short term inconvenience.
