It seems more of a UTI issue to me than a file extension issue.
There needs to be a way to manage
the precedence of the UTIs if multiple UTIs with the same extension
are registered. Alternately, if, as in
this case, the first UTI does not yield a successful preview or
thumbnail, then the next one should be
tried by QuickLook.
Is there a way to dump the overall UTI database to a text file so I
can see all the registered UTis with
their extensions? Is there a way to unregister the UTI for the disk
image?
I don't know of any way to unregister the existing .img UTI, but you
can dump the LS database with lsregister:
