I might be arm-twisted and tooth-pulled to work out the Cocoa stuff, but this is 10.5 only --- looking at my user stats for the past 2 months, over a third are still 10.4. So this is not going to work.
I was thinking to use NewMovieFromProperties and QTPixelbufferContextCreate as suggested by some other replies --- are these what's wrapped up inside frameImageAtTime anyway? They are available 10.3 or later. Will the performance be adequate (or are they wrapped cruftiness too?)

QTMovie frameImageAtTime:withAttributes:error: will do the trick very easily.
This method is currently way faster than using any crufty PICT code path and
should get even faster in the future. It also supports a wide variety of
"ImageTypes" that can be requested as output via the attributes passed in.
