I might be arm-twisted and tooth-pulled to work out the Cocoa stuff, but this is 10.5 only --- looking at my user stats for the past 2 months, over a third are still 10.4. So this is not going to work.
frameImageAtTime:withAttributes:error: works on 10.4 with versions of QuickTime that shipped after the 10.5 release. If you make sure and require a current version of QT (7.5 just shipped) with 10.4 you should be good.
I was thinking to use NewMovieFromProperties and QTPixelbufferContextCreate as suggested by some other replies --- are these what's wrapped up inside frameImageAtTime anyway? They are available 10.3 or later. Will the performance be adequate (or are they wrapped cruftiness too?)
You can certainly create an image type you require via the C API. Setting the movie time and producing a frame at time via an idle call is a valid approach as others have mentioned.
You may want to look at the QTPixelBufferVCToCGImage sample.
