I have found the problem.

Perian is not compatible with QuickTime 7.5 and produce this problem.

I've seen this bug and reported it to Apple before -

radr://problem/5770288 - Radar bug report
http://trac.perian.org/ticket/320 - Perian ticket

The obvious workaround would be to not signal droppable frames if the component is being opened for export. Unfortunately I can't find any way to detect this, and I don't have time for any reverse- engineering, so it may come down to adding a UI option to disable framedropping signalling.

I definitely can't see any reason why marking a frame as kCodecFrameTypeDroppableDifference should affect decoding that's not happening in realtime.

For H264, you can also remove Perian and use the Apple H264 codec that works well too.

Personally, I use a modified version of Perian that does not declare format already supported by QuickTime in Perian.rsrc.
