I'm getting the exact same results, on Mac OS 10.3.1 and QuickTime

I created a QuickTime movie with chapter titles following the
instructions at

(with slight modifications for QuickTime 6.4, as the instructions refer
to a pre-OS X version of QuickTime).
My problem is that the chapter titles are repeated in the pop-up box.
That is, instead of
"1. Opening credits"

"4. Baz"

I get

"3. Bar foo"
"4. Baz"
"4. Baz"

I found a reference to this problem in the list archives but the
solution suggested there (remove repeated titles from track
annotations) doesn't apply here. I assume I must have done something
wrong and that this isn't a bug... however, I don't know what could
have gone wrong.
Any suggestions?
