Indeed, the client communicates bank and forward with the server to determine how the stream is going. I don't remember exactly how the server determines how to take action. Reading through the setup file for the QTSS/DSS you'd be able to figure out a number of those processes. I think it's mostly based on latency and percentage of packets lost in conjunction with the packetizer settings used when the media was hinted.
Thanks for the hint on Thickening/Thinning. I did find a couple of posts in the archives on it and some other interesting things. It'll give me a start :-)

Don't ask me why I didn't have a look at the archives in the first place.
