I really don't know, but to set you in context of what QT seems to do I found out that if you encode a .m4a (AAC with iTunes) from a CD, iTunes will in fact attach meta data regarding the source. The problem is that this meta data is not accessible via the Annotations window in the QT Player. You can open the .m4a file (which I don't think is a compliant MPEG-4 file) with a Hex editor and see some of the meta data inside.

So, it may be that MPEG-4 does have support for meta data in some other structure different from "annotations" in QT. iTunes seems to get access to this meta data, but not QT Player.
