MPEG-4 Visual Simple Profile has not much in common with metadata. It's in
fact just a specification for the representation (compression) of video.
If you want metadata, then you should have a look at what is defined in
MPEG-4 Systems (look for Object Content Information - OCI). There you can
find support for metadata (like keywords, language descriptors, content
rating and classification, ...) and corresponding extension mechanisms (such
as hooks for MPEG-7 metadata).

I would call it a feature request ;-). It's just a matter of implementation
but that's not always that easy in practice... With regards to MPEG-4
Systems, QuickTime does not support that much for the moment (if we do not
take into account the support for the MP4 file format, but that's actually
another part of MPEG-4).
