You can do this with the C API or the QuickTime for Java API as long as you deploy the tool on a Macintosh or Windows server. Used a QT for Java class to extract image thumbnails from movies as part of a WebObjects application a while back but gave up as it was difficult to achieve a satisfactory, relevant frame to represent the movie.

The quickest way to do this, unless it already exists, looks to be to write a small Cocoa application and use QTKit to get the frame or better still get the poster frame. This would need deployment on a Macintosh Tiger server.
