I was referring to the format, but you're right...if rich SMIL support was a
requirement, then requiring the Real player might be a reasonable burden.
Generally, though, the requirement is for the spatial and temporal layout
capabilities that SMIL would've provided. Given that SMIL support is so
awful and inconsistent across media players, it's no wonder that it's
relegated to a curiosity.
The solution would be to write a SMIL player in Flash, in the same way that
DENG <http://claus.packts.net/deng/examples/> provides a standards-compliant
XML/CSS3/XHTML/XForms/SVG browser.

P.S. Anything other reasons that Real might be interesting? The format
will not make the leap to mobile, but I'm interested whether there's even
any valid reasons left to use it on the PC.
