Anthony,
This is one of the first interesting experiments I did with QuickTime 4.
Here's how I did it: Use two movies. The first ("Movie A") contains the
image plus a reference to the streaming audio, and is delivered that via
HTTP (or email or whatever). The second ("Movie B") contains the hinted
audio, delivered via QTSS/DSS.
(1) Encode hinted "Movie B" with audio, put on streaming server
(2) Copy image to clipboard
(2) In QuickTime Player, use Open URL to open "Movie B" stream
(3) Select All, Add to Selection and Scale
(4) Save result as "Movie A", put on HTTP server
(5) Let cool (optional)
Here's a Beastie Boys remix I did (audio takes a couple seconds to start
because I didn't trim it beforehand) that uses this technique:

Just wondering if anyone has done the simple thing of have a static image
along with a streaming audio track in a movie?
Thing that keeps happening to me when I add an image track (or a sprite
track) the movie then thinks it has 'time/duration' and nolonger streams.
