Here is the syntax that works.  It calls upon a routine named  "Start"
that is part of Windows.  The name of the file you call to open is
passed as a parameter to "Start"  the name of the file called apears to
be case sensitive and needs to be in all caps.

You put a text file named "AUTORUN.INF" and the file you open at the top
level of the CD.  The text file "AUTORUN.INF" contains the text:

[autorun]
open=start index.pdf

This works.  In the autorun.inf file the file name appears not to be
case sensitive, but the file name itself would only work for me when
written in all caps, i.e., INDEX.PDF.
