Oh, wow, how disturbing it is to hear of problems with either
approach. Since this code does all work for some people, how to
troubleshoot? One reason could be having the latest QuickTime, for
the codec. I see you have an older QT and 7+ is required. Another
could be waiting long enough for download, since they are many MBs
and high data rate, even though short clips. To see if anything works
for you, here is my latest QTVR, (of course the codec is old, so is
not a good comparison) with page code of the Eolas fix:

Hi Caroling,
I'm on Firefox 2.0.0.12, QT 6.5.2, W2K SP2.
My movies aren't QTVR though. Mark, can you tell me if you can play
one or either or both of these movies? One is code from Apple
exported as QuickTIme for Web. _http://wholeo.net_
This one gave me a QT warn telling me that it couldn't load the mov
file (or something like that, can't recall the exact words) then
froze Firefox, had to kill it. Second time I tried it didn't freeze
but there's jus a black screen with the QT controls underneath.
This one (clicking on one of the thumbnails there) instead of
giving the same black screen, shows a white screen.
Both movies actually play and have sound, just no image. Maybe
something CODEC related?
