Aaron,
I think you'll need to open other ports. The RTSP SETUP commands specify the port(s) the RTP streams are to be delivered on. Thing is, me thinks these ports can change. You could probably use tcpdump or some such sniffer to ferret out the RTSP SETUP requests from the packets the QT player sends DSS / QTSS, and then make sure those ports are open ... is there an easier way?
