Do you have a firewall between the QTSS / DSS and the clients?  If you do, are you streaming over port 80, 8000 (HTTP) or are you using RTP / RTSP (UDP)?  If the later, you need to open the RTSP ports (554 and 7070) as well as the ports the RTP streams actually come in on (6930-6999 I think?)  If earlier, you need to open 80 / 8000 / whatever port you're using there.  I'd search the archives of this list to be sure ... this question was raised by another recently.

If there is no firewall between the QTSS / DSS and your clients, i'd make sure they're both set to use the same transport i.e. HTTP vs. RTP / RTSP (UDP).
