Try a simpler qtaccess file:
require valid-user
There is no need for a <Limit READ> tag.

The qtusers files contains user names and encripted passwords (each
user in one line), I've added users using this command:
sudo qtpasswd -f /Library/QuickTimeStreaming/Config/qtusers <username>
When I open the URL in Quicktime Player I simply get the playlist
stream, no need to provide a username and password.
I use the latest version of darwin streaming server and OSX 10.3.4 ,
Quick time player 6.5.2
