He means the difference between <itunes:link rel="image" href="..."/>,
<itunes:image href="..."/>, <itunes:image>...</itunes:image>, and
whatever other variations people dreamed up while both your spec and
your app were changing every day in contradictory ways.
It's a valid question, and probably the #1 point of confusion for podcasters.

And yet despite being unsupported and undocumented, embedding in Atom
works just fine (as long as you don't care about being listed in the
podcasting directory, grr). I'm not sure how this could possibly be
construed as being worse than the situation of embedding in RSS, which
has been defined three times now, each one contradicting the last and
being posted in a new location which is only disclosed after some guy
who knows some guy who knows an elf overhears that a new revision is
available. The latest revision (which, by the way, still has
non-wellformed XML in its examples, and I don't mean RFC 3023) hasn't
been published at all but merely attached to a message to this mailing
list -- the Internet equivalent of putting it on display in the bottom
of a locked filing cabinet stuck in a disused lavatory with a sign on
the door saying "Beware of The Leopard".
