The 1-16.01 log shows something going wrong, and communication with the devices stops. After a while the hub driver hits fatal internal error and stops. It'd need to be rebooted to recover from this:

5367.959: [ 75577075]: USL - error - Param block - usbFlags not set correctly (std.) 1

I thought I'd fixed all of these in the later versions of OS 9.

Its not clear what went wrong. I might have a better idea if I had a bus probe, and hub verbose log*. That would also show me where the fatal hub error happened. If I could work out what the error was, it may not do you any good as there is no delivery method for any updates to OS 9 these days.

*To make a hub verbose log, use the USB Expert Options utility from the DDK. Set the logging level to 5, and check the "Hub Verbosity" box. You'll probably need to increase the log size as it will spew lots of messages like this.
