Er, yes it does work, with any window manager (after all, X events are
generated by the X server - the window manager has nothing to do with
it).  The --proxy-only thing helps with cut-and-paste focus, I think.  I
use quartz-wm as my window manager anyway, and have no problems with the
Ctrl-click and option-click for buttons 2 and 3 respectively.

Most of the time this isn't a problem, since I have a three button mouse
plugged in.  Three-button USB mice are very very cheap - why not just go
out and buy one?  It doesn't have to say "Mac compatible" on the box - I
use one of those tiny Microsoft USB optical wheel mice when I'm on the
road, and it works like a charm with my PowerBook.  When I'm at work I
use a Microsoft trackball.  It worked fine without the drivers
installed, although if you install the Microsoft drivers you can program
the extra buttons, which I find very useful.

Make sure you have "emulate three button mouse" switched on in the
X11.app preferences.
