I backed up my hard drive, did an erase/install, and used the Migration
Assistant to restore my personal files.

Eureka! It's DragThing. If I quit DragThing, I have the Cmd-Tab problem
also. While DragThing is running, Cmd-Tab works normally. DragThing has
a "Bring all windows to the front when switching" preference (intended
so that clicking on one window of an app brings all that app's windows
forward, like in OS 9), which I have turned on. Apparently, this
preference also affects Cmd-Tab switching.
