XFree86 = old and busted
X.org = new hotness

Licensing and development practices caused X to fork off of XFree86 into what is now X.org

Apple's X11 was (10.4 and before) based on old XFree86.  Now it is based on X.org.  Most of X on 10.5 is unmodified from X.org's 7.2 release.  The main exception to this is xserver which is heavily patched.  Hopefully once this development gets stabilized, we can roll the Apple changes into the master branch of xserver and do Xquartz development on the master branch.

From xdarwin.org:
    *  XDarwin: this server runs on Darwin systems without Quartz (i.e. non-OS X); not well-tested yet.
    * XDarwinApp: this builds XDarwin.app, which is a full X server using Quartz. It has loadable module support for AGL and CGL, and well as fullscreen and rootless support.
    * Xquartz: this server runs on Quartz-based systems, and is meant to work with X11.app
    * x11app: this builds a version of Apple's X11.app using patches by Torrey Lyons; most, but not all, functionality of Apple's original X11.app is present in this release. 

MacPorts is a ports collection (like BSD Ports) with a focus on Mac OSX.  It is basically a repository of install scripts, dependencies, etc.
