This may be some well-proven axiom in some RFC somewhere, but
it doesn't work for me. I frequently move the cursor "out of
the way" which puts it over some other (random) window. Having
my keystrokes going into the wrong window..., well, that's
what this is all about isn't it? If I put my cursor aside so
that I can use the keyboard to manipulate my application, I
don't wish to be concerned about where it has been moved aside
to, I wish to keep my brain focused on the current app without

You are talking about different things.

the (very rare) scenario where there is NO window manager. This
happens only during, for example, an xdmcp login, or if the
window manager crashes (e.g., due to a bug).

In normal situations, the focus policy depends on the window
manager, and (usually) your own preference. In the case of MacOS
X, usually this is quartz-wm whose focus policy is (AFAIK) not
configurable, and so if something strange like this (i.e., not
supposed to be supported by quartz-wm) happens I would say that
this counts as a quartz-wm bug.
