the (very rare) scenario where there is NO window manager. This happens only during, for example, an xdmcp login, or if the window manager crashes (e.g., due to a bug).
In normal situations, the focus policy depends on the window manager, and (usually) your own preference. In the case of MacOS X, usually this is quartz-wm whose focus policy is (AFAIK) not configurable, and so if something strange like this (i.e., not supposed to be supported by quartz-wm) happens I would say that this counts as a quartz-wm bug.
