Anytime you change repository information "behind Xcode's back," remember to use SCM > Refresh Entire Project or simply close and reopen the project. Xcode occasionally polls the SCM system to check for changes, but it doesn't do it that often.

I have, on occasion, had Xcode and my Subversion repository get permanently out-of-whack for no reason that I could discern. I have successfully fixed all coordination problems by making sure everything is checked in, trashing my entire project folder, and rechecking everything out from version control. It's fast, simple, fixed all of your problems, and probably isn't a bad thing to do from time-to-time anyway.
