SCM > Refresh Entire Project doesn't work real well for us with Subversion (I think it works better with CVS). Currently, if you want to use Subversion and Xcode, you're going to have to do some of the work outside of Xcode. (We've filed a number of bugs, so hopefully this will change in the future.)
