Further testing shows that it's only one application in which I see this
behavior -- a Cocoa application that I wrote myself. In other apps, the
Apple menu's menu items seem to show up even if they haven't previously been
exposed. I am able to see my own app's Apple menu menu items after I've
opened it once.

What exactly is the bug? That the Apple menu has no children until it's been
opened once? -- at last in my app?
