I tried to get someone from our group appointed (Sussex Mac User Group, in England) and got a deafening silence. We don't maintain membership databases as we effectively only exist as an email list (so the membership list is in the Google group). When I try to log-in to one of the addresses in that pdf, I am told my apple userid password has expired (works on iTunes!).
Frankly not sure I have time to chase this up but if there's an easy way to get someone registered, I'd be grateful for pointers. I am afraid I don't remember precisely which email address at Apple I emailed about a year ago, but it was every relevant one I could get my hands on -- and none even replied.

This person may order the resource mailing and is considered Apple’s primary contact for that group. One of their responsibilities is to " deal with individual requests for coupon codes." Please review the User Group Ambassador Guide (PDF)
