OK while I hate to admit that I'm an idiot, it may be true in this
case...doh!. I had my card reset today and now everything works ok
again using my PIN. I could swear that I typed my PIN in before, but
it wouldn't take it...thats when I started trying my password.
Anyway, false alarm...all is good again...at least for now :-)

I just upgraded to 10.4.5, and now my smartcard shows that it is
locked. When I try to unlock it, it asks for my keychain password. I

I must have missed this. It was asking for your smartcard PIN, not
your login keychain password. You locked your card after the third
attempt.
