Buy a CCID compliant reader. I've got an SCM SCR3310 that worked out
of the box with no issues.
The "DoD Configuration" FireFox add-on will configure FF for you:

I did a code walk & have had discussions with the author, and I'm
pretty confident it's not doing anything hokey.
You *may* have FF crash-on-startup issues. If so, launch FF without
the card in the reader. You *may* have PIN prompt issues (i.e., the
PIN prompt never appears). If so, you can log into the CAC manually
via the Security Devices dialog (Preferences | Advanced | Security
[or is it Encryption?]).
Safari works too, BTW. Not Camino (yet; there's been some motion on
fixing this this week, so I remain hopeful).
