I was having a similar problem but on only one out of three of my Macs. The Macbook Pro at work and my iMac at home both worked as expected. However, when I got out my Macbook to prepare it for a trip I'm on this week it wouldn't connect to web mail at all - I almost immediately got the 404 error and Safari never even tried to unlock the CAC (i.e. I never got the CAC password dialog box). I did all the things you did and never got it to work at all. I even tried connecting with Firefox to 1) verify the web mail site was working and 2) to see if unlocking the CAC that way somehow magically made Safari work. No joy. This morning, when I tried Safari again to verify the error message it worked and has been working for about 10 minutes. I haven't made any changes to the system between when it stopped working and when it started working. I had upgraded 10.4.9 just before it stopped working.

I was happily using Safari and the latest CAC enabled web mail (AFMC)
just fine for a time then lately after a viewing a few messages in
OWA I suddenly get the dreaded 404, authorization required web page.
I have to empty the Safari caches, quit Safari, remove and reinsert
my CAC card into the CAC reader (SCR331 with latest firmware), unlock
the keychain, start Safari and finally log into the AFMC OWA server.
And I have to do this pretty much every time I read more than 2 or
three emails. I used to just click on the "Inbox" icon to refresh the
list of email to see if there was any new mail. But now I just get
the 404 page.
I recently updated to 10.4.9. Thinking that might be the problem
I reinstalled the combo updater, trashed the Safari plist file in my
home folder's Preferences directory. Still have the problem.
Is anyone else having this problem?
I am running 10.4.9 on a MacBook Pro. Thanks
