There's the top-level entry, and then there's an entry under it for
each reader. Select the reader and check the "Status" entry. When
the card is not detected it will say "Not Present." If the card is
inserted but it says "Not Present," then pcsc is likely to blame.
Since you've re-registered the card with the bundle, you can try
unloading the PKCS#11 module and re-loading it (you'll have to do
this by hand as the DoD Config add-on isn't friendly like that) and
see what happens.

Then followed the FF instructions from above. FF now recognizes the
CAC/Reader. Inserted the card, clicked the "Log In" button on the
"Device Manager" Dialog, entered my PIN and it seemed to like it. I
then closed all those dialogs, and then wen to https://
webmail.afmc.af.mil/Exchange at which point FF asked me to select a
cert (I selected my identity) and I get the 401 error. I tied again
with my email cert and I get the 403 error.
Tried again with Safari but still get the 401 error.
