I have a similar problem. When I first got a CAC reader at home, I was able to plug in and start logging in immediately to my AF Portal, etc. However, I recently got a new card and now Safari and keychain Access does not recognize my card while in the reader. The green light on my reader flashes as the USB drive recognizes there is something there and its getting power, but nothing changes when I put my card in it. I have restarted the programs and the computer; I went to the Apple store to see if it was a problem with keychain and they couldn't find an issue with that either. I've also tried Firefox as well.

This isn't an issue with the CAC. It's a known issue with Safari 3 and occurs with a non-CAC login to AKO, and AKO's non-answer is to not use Safari 3. I know they are aware of this issue, but I don't know of any specific plans to address it.
