I had the same problem and just go it to work. I was playing around
with different identity preference settings and was having the same
issues. I just deleted the identity preferences for webmail in
keychain to start fresh. I used https://webmail.east.nmci.navy.mil/exchange
without the "/" at the end (I'm on an east coast server).
It now works. I don't know if the "/" was this issue or just starting
from fresh did it. I also quit both keychain and Safari before trying.
I'm using MacOS 10.5.4 and an omnikey reader with an Oberther CAC.

I just checked the AF web mail again, found where all of my identity
preferences were visible, deleted them all and tried to let Safari let
me access Air Force web mail without an identity preference.
For me, this did not work. I get an immediate access denied.
I had to add back in an identity preference in order to get access to
my account. Then I get the CAC pin prompt and all is well, at least
for the first several minutes of access.
Using the an Omnikey reader and a very recent Oberthur card (ID V5.2e
Dual)
The AF email is not based on nmci as far as I know, but perhaps it is.
