Bear in mind that Apple lags pcsc-lite by a fair bit.

Tim,
Not a true statement anymore... If you list the version of pcscd via
terminal you will only see 1.4.0, but Apple had to fork from
incorporating a pure pcsc-lite source because of the integration with
Securityd.
Also note that both David Corcoran and Ludovic Rousseau are members of
my Development Team at the SmartCardServices project on MacOSForge.org
which is THE Smart Card source for what is in Mac OS X. Apple will
be incorporating snapshots of source from this project into future
releases of Mac OS X. We are taking that project much further with
direct involvement. Ludovic Rousseau maintains the source for pcsc as
David has moved on to many other things.
Mac OS X 10.5.6 was a *reset* of this condition... as you all recall,
that is when all of the Smart Card Services changes were
incorporated. Versions of 10.5.0 - 10.5.5 had the issues... unless
you installed my SCSU v1.2 on 10.5.4 or 10.5.5.
