It sounds like there really isn't a good option for large scale app deployments out there. I would also be fine if I could just have one account and be able to re-buy apps for each phone.

We currently don't support purchasing applications. Users are expected to create personal iTunes accounts, and are allowed to purchase or download apps to the phone from/with their personal account as long as the application meets mission needs.
It is theoretically possible to buy "gift cards" for accounts, but that involves a government purchase being applied to a user account, so not a great solution. We've talked to Apple about alternatives (would be nice to have two accounts for a phone), but haven't found anything we like.

We're in the beginning portions of an iPhone pilot. We have been creating individual iTunes store accounts for each phone, but it turns out that you can have more than 5 accounts tied to one credit card. I emailed the main iTunes help email and they didn't know of anything.
