"iPhone OS" (the OS that runs on iPhone, iPod touch, and iPad) was renamed "iOS"[1]. iOS by itself doesn't mean anything other than the OS family; I presume therefore that you are talking about iOS 4.0, due for release today.
iOS 4.0 doesn't have any specific known issues that would make me shy away from upgrading; it has been available in its final form since 7 June to registered developers, and there haven't been any "showstoppers" discussed.
