It's more complicated than this. ISO/IEC 7816-3 covers the electrical requirements for smartcards, and it allows up to 5V. The majority of cards (including most CACs) run at 5V but the trend over time is toward 3V smartcards (type B, IIRC) because they're more reliable.
But from a card *reader* manufacturer's POV it's not sensible to produce a 3V-only reader because doing so is just begging for "my card won't work" calls and returns. Not how you want to run a business.
The potential white knight remains Bluetooth, but without the HID profile on iPhones and iPods, the options are more limited (and even more burden is shifted to the developer, reducing profit).
