I've been using Thursby's reader for a few months now.
Just like Lee Zimmerman mentioned, it works very easy.
I've accessed the following websites all via CAC on my iPhone 4S:
Army Knowledge Online (both regular and webmail)
Outlook Web Access for the Army Reserve and DoD enterprise Email
DFAS' MyPay
DTS did not work, I'm assuming because of its need for Java, but I couldn't imagine trying to do my travel voucher on my tiny screen. :)
Like someone else mentioned, all CAC readers are not the same. PKard for iOS does work with 3 readers, of which only 2 are on the market right now.
I am very happy to see a product like this finally available for people who want and need it. Not sure why it took so long for Apple to approve the app.
