Tactivo has both an iPad and iPhone version for the 30-pin connector. Every accessory vendor is working on a lightning version.
MilitaryCAC.com has a nice overview of smartcard readers for iPad and iPhone both in-production and pre-announced, as well as information on Android, Surface etc.

Chief Danberry's site focuses on CAC but it's broadly accurate for two factor strong iOS authentication with PIV, PIV-I, CIV, .NET smart cards.
