Which OS version?
10.4.10 and 10.5.
Try enabling DiskArbitration logging to see if the eject makes it that far.
This is what appears in the log on 10.4.10: 08:56:47 loginwindow [120]:16131 -> diskarbitrationd [78] 08:56:47 unable to queue solicitation, id = 004D6967:0035CF70, kind = disk unmount, disk = /dev/ê (status code 0xF8DA0003). On 10.5: 09:18:53 loginwindow [28]:18179 -> diskarbitrationd [36] 09:18:53 unable to queue solicitation, id = 0000000000491761:0000000000598960, kind = disk unmount, disk = /dev/ (status code 0xF8DA0003). 09:18:54 0xF8DA0003 works out to be kDAReturnBadArgument.
