Thanks for the suggestion. Unfortunately, it did not do the trick for
me. I am using quartz-wm. I did have a .Xauthority file in my home
directory, however it was empty. Nevertheless I moved it aside, but it
did not help.
As you suggest and as my experience with the problem gradually getting
worse also suggests, it may have something to do with crud
accumulating in the system. Unfortunately I do not know where to look
for it.
After copying it from /private/.../xinitrc to .xinitrc in my home dir,
I now have that file. For what it's worth, I added a line in .xinitrc
to start xeyes. That works fine. I also changed the line that starts
xterm to be xterm -hold &. Now when X11 starts the first dead xterm
window does not disappear. It is left empty except for the cursor
frozen in the upper left corner. The absence of text in the window
tells me that it did not get as far as reading my .tcshrc file. All
that does is define a few simple aliases and then echo 'done'.
This leads me to guess that the problem has something to do with the
interaction between xterm and the shell, but I don't know where to go
with that.

I've read about this, and then had the problem myself just yesterday.
I'm not using the built in window manager quartz-wm, but rather am
using another
(xfce) that saves info about the disposition of windows for use when
restarting X11. I deleted those files, along with .Xauthority,
restarted
X11, and everything was fine. If using quartz-wm, you might just
try deleting .Xauthority.
