I too have had this problem, but usually when I 'su' to another account.
By making sure the xterm stays up if the shell process dies, I was able to see
an error message, but I don't remember what it was. If I remember, it wasn't
very helpful.
Actually reading your message again, my situation was slightly different. The
shell would come up for me and I could do some things, but then it would
randomly close the xterm without warning.
Perhaps you can experiment by using a different default shell; looks like you
are using tcsh.
