Sounds likely. If you're not using ssh all the following need to be
done:
1) In X11.app's preferences, Authenticate connections and Allow
connections from network connections (especially the latter) need to be
checked.
2) You need to configure the firewall in system preferences to pass
port 6000. Everyone does have the firewall switched on, right?

Within a trusted network (and his VPN is presumably encrypted, so
security is no longer an issue) rsh is the right way to go. ssh would
add an extra unnecessary layer of encryption, which would be a waste of
time, and make the performance suffer.
ssh still gives you the convenience of doing all that xauth rubbish
without needing to allow .rhosts access.
