Philip,
Do you use fast user switching on your machine, or was there someone else
remotely running an xterm on it when you started X11.app?
I submitted a bug to Apple last August that has a similar signature to what
you describe if you try to use X11 in two user accounts simultaneously. I
have included the text of my bug report below, including a way to reproduce
the result.

Summary:
If an xterm is run from two accounts on the same Mac, the second xterm will
lose the ability to cut and paste between windows using the select/middle
mouse button method. If the second account is working using X11.app and
quartz_wm on the console, that user may also lose the ability to switch
between X11 windows using the mouse.
Steps to Reproduce:
1. Log onto a Mac, start X11.app, and open a couple xterms.
2. Using Fast User Switching, log onto another account, start X11.app, and
open a couple xterms.
3. In the second account, select some text in one of the xterms.
4. Paste the text into the same xterm using the middle mouse button.
Expected results:
The text should paste into the xterm.
Actual results:
The text does not appear in the xterm. Moreover, attempting to select
another xterm by clicking on it's title bar will not work.
Workaround:
Bringing another application to the foreqround will cause the text to
properly paste into the xterm.
Isolation:
The same situation occurs if a user on a remote machine starts an xterm on
the Mac that displays at the remote machine, and a user on on the Mac is the
second user. In other words, the problem does not appear to be tied to Fast
User Switching; it appears to be tied to the X implemetation. This problem
is repeatable between Macintosh configurations also.
