You're not the first one confused by this (nor probably the last).
It is off by default. You can turn it on by moving the X11 executable
(found in /Applications/Utlities/X11.app/Contents/MacOS) out of the
way and puttig in its place a shell script that calls the real
executable with the argument "+xkb" (it seems entirely wrong, since it
X + turns things off :).
I tried this, and after the invoking the same setxkbmap I've
used happily for years in the UNIX world the keyboard became
completely unusable. So I'm not sure exactly what they have changed,
but I've found Xmodmap more reliable.
