The best way I've found to reproduce it is to do the following:
1. Install Vim.app from
http://macvim.org/OSX/files/binaries/OSX10.4/vim7.0.224.tar.bz2
(un-bzip2/un-tar and copy Vim.app to /Applications)
2. Run /Applications/Vim.app/Contents/MacOS/Vim to edit a reasonably large text file
3. In vim, do ":set mouse=a"
4. Scroll back and forth really fast using the mouse wheel.
X11 should freeze up within 5-10 seconds of doing that. It does it for me every time.
I can reproduce this both on my MacBook Pro 2.16GHz Core Duo at home as well as my iMac 2.16GHz Core 2 Duo at work. I have not been able to reproduce this on any PPC Macs.
One of the other reports I heard had limited this to logitech mice, but I am able to do it using the built-in trackpad on my MacBook Pro, so I think that report is false.

This is another example of a bug about which we have received
countless reports; Most often, the hang happens while scrolling with
the mouse wheel in XEmacs. Unfortunately, I've spent about an hour
trying to reproduce it, and I can't make it happen. So, yes, I
believe you (and everyone else) when you say it's happened, but
without being able to see it first-hand, I can't fix it. :(
